Enrolment is quick and easy. We offer two methods of enrolment for your convenience. You can enrol either online or call the resident representative over the phone.

Registration Process

After receipt of the dully filled in enrolment form, the following documents would be required to be completed and signed:

• Choose your choice of Payment Options;
• Medical and Health History Profile;
• Informed Consent- Release from Liability;
• Informed Consent- HIV Testing; and  
• Client Service Agreement.

Once the documentation and the payments are received at PathCare’s Cord Blood Facility you would be designated with a unique BAR CODE NUMBER and the collection kit containing the BAR CODE Number will be dispatched to you for collecting the cord blood, at the time of delivery of your baby.
